All costume rentals require a completed RENTAL AGREEMENT.
Some rentals, including most uniforms, also require a letter of RESPONSIBILITY, a CERTIFICATE OF LIABILITY INSURANCE, and a LETTER addressed to our business describing the action of the uniform, and the date, time, and location of use.
If your project is tax exempt, please send us that form as well.
